Eligibility for Hospice Care
- Based on the clinical judgment of the patient’s Primary Physician and the Hospice Medical Director, it has been certified that the patient has a limited life expectancy.
- The patient’s Primary Physician authorizes Hospice care.
- The focus is now on physical, emotional, and spiritual comfort rather than curative measures.
Where are Hospice services provided?
Hospice care is provided wherever the patient is residing.

St Sofia Palliative and Hospice Care, Inc. offers 4 levels of care to meet the individual needs of each patient. Each level is designed to accommodate the patient’s acuity level and assist the family during a crisis.
